Annual Student Employment Office Audit Checklist

Every year the Student Employment Office conducts an audit of on and off campus Student Employment records. These records are required by federal mandate to include the following:

_____ Individual personnel file for each Student Employee.
 
_____ Check-in forms for new hires at UCCS (1st UCCS job is with your dept.)
 
_____ Check-out forms for terminated employees.
 
_____ Class schedule for every term worked (Workstudy students).
 
_____ Compensation Plan which details the initial hiring wages and conditions for raises for all student employees in the department. (One copy of this form should be supplied to the Student Employment Office and another kept by your department). The department copy should be posted in a public place for your student employees' perusal. It is recommended that you give a copy to each employee as a part of their orientation to the job.
 
_____ Evaluations for all employees who have worked for you for twelve (12) months, who have received a change in status or rate of pay, or who have been terminated for cause.
 
_____ Job Data Worksheet (JDW) copy for the most current term employed (originals go to SEO).
 
_____ Job Description signed by both the supervisor(s) and the student employee.
 
_____ Sign In/Out sheets, with student and supervisor signatures, that note the exact dates and times student employees work each day (In date order).
 
_____ Time Report Rosters (TRRs - copy) that have been submitted to the UCCS Personnel Office for every pay period the student is employed (In date order). Off-Campus Work Study Employers: submit TRRs to Student Employment, Fax#: (719) 262-3650.
