As a prospective student-athlete, some coaching staffs will ask for your participation in a tryout. If this is the case, then the forms below, both the "prospective student-athlete tryout form" and "prospective student-athlete medical disclosure form", need to be completed and returned either by fax or email. This needs to be completed at least 24 hours prior to the actual tryout date (i.e. if the tryout is on a Thursday, then the forms need to be to the department of sports medicine on Wednesday). Any individuals that do not have their tryout forms available 24 hours in advance will not be able to participate in the scheduled tryout. When being recruited and/or participating in a University sanctioned tryout , a full medical disclosure needs to be completed and returned to the University of Colorado at Colorado Springs Sports Medicine Department. The link below is the form that must be completed and sent to the University of Colorado a Colorado Springs Sports Medicine Department before coming to campus for an official visit and/or try-out.
