Every student-athlete (returning and new) at University of Colorado at Colorado Springs (UCCS) must be NCAA and medically eligible to participate in any sport at UCCS.  This process takes several steps.  The steps below are the actions, forms and/or questionnaires that must be finalized before the student-athlete is medically cleared and allowed to participate in practice and/or events.  Also below is some information on physicals and a letter that describes medical eligibility requirements.

This year listed with all forms (at the end of each sections) is a link to an interactive form "(emailable form)".  This form can be typed in and printed and/or typed in and email to the Sports Medicine Department  If you chose to email he informatin just remember that some of the information that is contained on these forms is personal medical information.  With that in mind, we (the University of Colorado at Colorado Springs and its entities) cannot guarantee the security of that document over an open internet.  If emailing is the best option for you, then we would suggest that you provide all the information except for the SSN# or any sensitive personal medical history.  This information can be provided upon coming to campus.  The other drawback to this option is official signatures, all signatures, including the parent/guardian for those under 18, MUST be completed to be able to participate.  The forms can be signed upon showing up to the University.

 

1.  Incoming (new) student-athletes must complete a pre-participation physical provided by the sports medicine department of University of Colorado at Colorado Springs.

2.  Medical History Form must be filled out and turned into the sports medicine department


UCCS General Medical Information Questionnaire (standard)  -  This form needs to be filled out by everybody that participates in athletics at University of Colorado at Colorado Springs.  It provides the sports medicine staff (Certified Athletic Trainers, Physicians, etc.) with information needed about emergency contacts, basic insurance information and contact information. UCCS Freshman or Transfer Student Athlete Medical History Form (standard)  - This form needs to be filled out by every new student athlete at UCCS, this includes freshman and any transfer students. It provides the sports medicine staff with information on injuries and medical issues that the student may have had prior to entering UCCS's Department of Athletics. UCCS Returning Student-Athlete Medical History Form (standard) - This form needs to be filled out by all returning student-athletes to UCCS.  If you have competed at UCCS within the last year (12 months) you should complete this form. It provides the sports medicine staff with information on injuries and medical situations that may have come up in the last 12 months. (emailable form)


3.  An Assumption of Risk, Consent to Treat, UCCS HIPAA and NCAA HIPAA Form must also be filled out and returned to the sports medicine department.

UCCS Assumption of Risk, Consent to treat UCCS HIPAA and a NCAA HIPAA Form  -  This is a multi-purpose form that informs the student-athlete of the inherent risk associated with participating in any athletic activities within Colorado at Colorado Springs.  The subsequent sections are permissive, they allow the sports occur during an intercollegiate activity as well as allowing the sports medicine department to disclose information about those injuries to the individuals listed on the form and only those people.  This form also includes an NCAA HIPAA form that allows the UCCS Sports Medicine Department to keep medical file on a secure server at the NCAA Headquarters as well as allows the NCAA to perform random drug testing to any student-athlete. 4.  A Parent's Insurance and Insurance Notification Form must also be filled out and on file in the sports medicine department.

UCCS Parent's Insurance and Insurance Notification Form  -  This form is to filled out and returned to the sports medicine department before season begins.  Due to some of the information it might be easier to fill it out prior to leaving home or returning to school.  It allows the sports medicine department to keep tabs on the insurance needs of our student-athletes.  Without this information on hand in an emergency situation it will be difficult to have the student-athletes medical needs met in a timely manner.  For more information please refer to "Insurance Info" on the menu bar to the left and specifically the "Information Letter." 5.  A Nutritional Supplement Disclosure and Review Form must also be filled out and returned to the sports medicine department.

UCCS / NCAA Student-Athlete Nutritional Supplement Disclosure Form - This form if filled out properly ensures that student-athlete are not using NCAA banned substances that might cause them to test positive in a randomized drug test that will significantly effect their athletic eligibility. 6.  A Student-Athlete Nutritional Health Questionnaire must also be filled out and returned to the sports medicine department.       

UCCS Student-Athlete Nutritional Health Questionnaire  - Due to an increase in nutritional concerns and  women's health issues we also ask our female student-athletes as well as our male distance runners to fill out a questionnaire that can be used to help determine possible nutritional concerns or a history of injury that might be associated with nutritional concerns. 7. Student-athletes with a history of allergies and/or asthma also need to complete the Allergy and Asthma Form.  This has been instituted to get a better grasp on the needs of this special population

UCCS Allergy and Asthma Questionnaire  - This questionnaire only needs to be completed if you have or have been diagnosed with EIA,EIB,VCD or any other form of allergies. The ability to provide adequate medical supervision to these individuals is imperative, so we need all the information that we can to provide those services. (emailable form)
