PIPES Student Research Enrollment

Thank you for your interest in enrolling your child in our PIPES Research Program! You have two options for enrollment - online and paper.

Online Enrollment

We use PsychData, a secure online webservice to collect data for PIPES enrollment. Parents will need to create a login account and complete their part of the enrollment, and then direct students to login to this account to complete their portion. Parents will need to create a separate account for each child enrolling in the research program. Once we have received your enrollment information, we will notify you of upcoming events. Once you have completed the online portion of the PIPES Research Enrollment, please download the following two forms, complete, sign and return them to us at the address below.

Medical Release Form
(only required if your child will be attending workshops or camps)

Authorization to Release Records
(a required form for gathering research data)

Paper Enrollment

To enroll your child in the PIPES Research Program, please print, complete and return the following parent and student forms to:
