IT Account Request

  • All students, staff, and faculty get an IT account automatically. Student accounts are created after they register for their first class. Staff and faculty accounts are created after they are added to payroll by the Personnel Department.
  • There are instances when an additional IT account is needed for email or a website. An addition IT account is not authorized for storage space. Click the link below for shared storage space for a department or university organization.
  • Here are some examples of valid requests for IT accounts.
    • 1. A university club wants an email account and/or website
    • 2. A department wants a website on the Unix server
    • 3. A staff or faculty member sponsors a regular event requiring IT login
    • 4. A staff or faculty member retires and wishes to retain their email account
    • 5. A unpaid staff or faculty member needs an account; e.g. ROTC or volunteer
  • To request an IT account, click on the link below. Print out the document.
  • All lines refer to the sponsor's information except for the personal information in the middle of the form. Retirees act as their own sponsor.
  • The information in the middle of the form applies only if the account is for a person (example 4 and 5 above). Otherwise leave it blank.
  • The sponsor signs the form.
  • Fax the completed form to Brad Cook at 262-3592 or bring it to the Help Desk on the first floor of the El Pomar Center.
