Departmental Forms for Admitted Students:

• Admission to Candidacy
• Diploma Card
• Examination Report Form (Masters)
• Examination Report Form (Ph.D)
• Transfer of Credit Form (used for those students transferring in courses from other Universities)
• Request for Unclassified Credit (used for those students who have taken courses at UCCS before being formally admitted)
• Course Validation Form (used to validate courses that are more then 6 years old)
• Approval of Format (Thesis/Dissertation Form) Only for those students Filing Thesis/Dissertation to Library
• Request to Participate in Commencement
• Concurrent Registration (for students who are enrolled at other CU campuses). See Admissions & Records for form
