Scholarship Application

Selection Criteria:

The HA/DR Scholarship is available to individuals who are unable to pay all the costs necessary to participate in the HA/DR Technology Workshop May 25-26, 2011.  Applicants will be asked to propose a reimbursement amount they will require for registration, lodging, and travel costs. Travel costs will be reimbursed upon receipt of a detailed, signed reimbursement request form (to be provided to scholarship recipients in advance of the workshop).

The scholarship committee will meet in mid-May 2011 to select the recipients.  Recipients will be notified immediately via e-mail, followed by a notification letter with the necessary forms to be completed.  All applicants will be mailed notification letters from the Workshop office by the week of May 13, 2011.

The Scholarship Committee will review all eligible applications and base their recommendations on expressed financial need and benefits the candidate expects to receive from participating in the Workshop.

Scholarship applications must be completed no later than Friday, May 13, 2011. Only emailed applications will be accepted - faxed or mailed applications will not be eligible for consideration. Please email scholarship application to

Conditions of Award:

Scholarship funds are applied in the following priority:  registration, lodging, and travel.

The recipient is responsible for the balance of transportation and per diem expenses.

The recipient must submit the request for reimbursement no later than 30 days following the completion of the Workshop, or funds may be forfeited.

Scholarship recipients will be asked to serve as a volunteer live-blogger or micro-blogger during breakout sessions for one day of the Workshop.  In your application materials, please be sure to indicate if English is a second language for you.
