This form can be brought into our office or emailed to our office at firstname.lastname@example.org. If emailing form, save form as LastName_FirstName.pdf.

Changing Majors or Institution?
Changing majors at UCCS or changing schools within the St. Louis Region, please fill out the appropriate form and email it to email@example.com.
Changing schools from outside the St. Louis Region, please fill out the appropriate form through VONAPP.
Attending More Than One Institution?
If you plan on attending another school while simultaneously attending UCCS, you will need to complete a parent school letter. The form can be accessed below.
