Upon completion of the required courses, participants are required to submit the following for certification:

1. Portfolio for review. (Portfolio should include the following: One sample of written work from each class; personal statement reflecting upon your own growth throughout the program). Maximum 10 pages.
2. $100 certification fee; please make check out to "UCCS Matrix Center";
3. Completed evaluation and suggestions for program development. Click here to download the evaluation form.

Allow four weeks for certificate to be mailed to you. Transcripts may be requested at any time on the course registration website (click here for details).
