Please report cases in which your visiting J-1 scholar’s appointment at UCCS will end more than 30 days before the end date listed on the DS-2019 form.

The completion date on your I-20 is an estimate. If you will not complete your program objective by that date, you must request an extension prior to the I-20 completion date. The U.S. Immigration regulations require that I-20 remain valid during the course of study.

J-1 Exchange Visitor Insurance Compliance Form

U.S. federal regulations require J-1 Exchange Visitors and J-2 dependents to carry health (medical) insurance during the entire length of their program. Willful failure to carry and provide proof of adequate health insurance will be considered to be in violation of federal regulations and subject to termination as an Exchange Visitor participant.

This form and instructions will lead you through the procedures for getting your foreign visiting scholars SEVIS visa documents and assuring the University is in compliance with the SEVIS system and regulations.

F-1 Financial Statement Form

F-1 students need to show proof of liquid assets sufficient to pay for their entire first year of education and living expenses, as well as proof of readily available funds to cover the remaining year(s) of studies.

J-1 Exchange Student DS-2019 Request Form

Departments should please complete this form for newly-admitted international exchange students of UCCS.

Please use this form to request a lost I-20, an I-20 for a dependent and/or child, for updated degree/funding, or other reasons.

Transfer In Form

Please use this form if you are an F-1 or J-1 student transferring to UCCS from another U.S. institution.

Transfer Out Form

Please use this form if you are on F-1 or J-1 visa and transferring from UCCS to another SEVIS school.
