Instructions for Submitting a Request


The following tutorial will guide you through submitting a service request using the IT self service page which can be accessed via the button below.


    • Click on Login.



    • Login using your UCCS username and password. You will need to type "UFP\"before your username.



    • Hold your mouse over Requests & Forms and then select New Requests & Forms.



    • The following page will appear.
      1. Select a request item from the Featured Forms list. A form will appear below that will ask for specific data regarding your request.
      2. You can upload pictures or documents with additional data regarding your problem.
      3. Click on the Save Incident button before you logout or leave the site.
      4. Click on the Logout button when you are finished.



    • Once you have saved your incident, you will be given an incident number. This number can be used to view the status of your service request in the future.
