Students’ Rights and Responsibilities

• Students have the responsibility to identify themselves to Disability Services and qualify for testing accommodations.
• Students have the responsibility to initiate requests for testing accommodations by giving the professor the Disability Certification form within the first two weeks of each semester they are attending classes.
• Students have the responsibility to make the necessary arrangements and follow the Disability Services Testing Policies and Procedures when an exam is proctored by Disability Services.
• Give the Test Accommodation Form to your professor one week before each scheduled exam. (Fill in Part I of the form)
• One week before each scheduled exam, call (255-3354) or stop by Disability Services (Main Hall #105) to make an appointment to take the exam. Please have the course number, your professor’s name and contact information.
• Tests must be scheduled at the same time as the class meets, unless the professor has approved the exam to be taken at a different time. Special arrangement must be made for night classes.
• Students have the responsibility to follow all University Policies and Procedures. (Link to Testing Policies Form and link to Academic Dishonesty Policy)

Professors’ Rights and Responsibilities

• Professors have the responsibility to include a “disability accommodation statement” on the course syllabus. A sample statement is: “ If you have a disability for which you are requesting an accommodation, you are encouraged to contact Disability Services within the first week of classes. Disability Services is located in Main Hall # 105. DS phone # is 255-3354.”
• Professors have the responsibility to deliver tests to Disability Services when DS is providing the extended time and proctoring the test. Tests may be delivered in one of the following three ways:
• Drop off the test at one of the designated drop-off points:
  • CoH 2025
  • Eng 201
  • Science 266
  • DH 137
  • MH 105
• E-mail the test to Disability Services at dservice@uccs.edu
• Fax the test to Disability Services. Fax # 255-3195.
• Professors have the responsibility to maintain student confidentiality
• Professors have the responsibility to provide clear proctoring instructions. (Fill in Part II of the Test Accommodation Form and submit this form with the test)

Disability Services’ Rights and Responsibilities

• Disability Services has the responsibility to determine reasonable testing accommodations as supported by the submitted documentation.
• Disability Services has the responsibility to pick up exams from the designated drop-off points
• Disability Services has the responsibility to proctor tests and report any suspected academic dishonesty to the appropriate professor. (Link to Academic Dishonesty Policies)
• Disability Services has the responsibility to deliver tests, in a timely manner, to designated drop-off points
• Disability Services has the responsibility to maintain strict student confidentiality.

Disability Services has the right to deny testing accommodations that are not supported by qualified documentation.
