All the forms that need to be completed can be found online at IM Leagues. Below is a list of the forms that each participant must complete within the website before they are able to compete for a club. For further questions regarding required documents, either look under the FAQ section to find more information about the IM Leagues website or contact Mallory Price at mprice@uccs.edu, or by phone at 719.255.7513.
- One completed and signed Waiver
- One completed and signed Travel Acceptance
- One completed and signed Athlete Code of Conduct Form

All Club Sports participants will be required to create an account on IMLeagues.com.
To create an IMLeagues account:
1. Go to www.imleagues.com/UCCS/Registration OR
Go to www.imleagues.com and click Create Account OR
Go to www.imleagues.com and click “Log in with Facebook” if you have a facebook account with your school email attached - this will automatically create an account, fill in your info, and join you to the school (steps 2-4)
2. Enter your information, and use your School email if applicable (@uccs.edu) and submit.
3. You will be sent an activation email, click the link in the email to login and activate your IMLeagues account.
4. You should be automatically joined to your school – If not you can search schools by clicking the “Schools” link
How to sign up for a Club Sport:
1. Log in to your IMLeagues.com account.
2. Click the Player Sign Up button at the top right corner of the page. Click Join Team button at the top right of your User Homepage page.
3. You will see an option for Intramurals or Clubs. Choose Clubs.
4. Choose the Club Sport you wish to join.
5. Choose the team you wish to join.
6. Compelte the three required forms found in Red at the top of your user page.
