• REPAYMENT AGREEMENT FOR PAST DUE ACCOUNTS (Adobe PDF)
  Balances for a semester become PAST DUE after the final payment deadline has passed as described in the Main-Campus Billing Overview. Once your balance becomes past due, you will need to submit this monthly repayment promissory agreement to avoid any further collections action. Financial stops will still be active until your balance is paid-in-full.
  
  

• CHANGE OF ADDRESS FORM for STUDENTS IN COLLECTIONS OR PAST-DUE REPAYMENT
  If your account is currently in collections, or you are repaying your balance through Student Debt Management, you can use this form to update your address with SDM to ensure that you are receiving all of the important documents that are being sent to you. This form will ONLY update your address with SDM, and will NOT affect the address on file with the University. If you also need to update your address with the University, you must contact Admissions and records at (719)262-3361, or use the Student Online Center above.
  
  

• DUPLICATE ITEMIZED RECEIPT REQUEST FORM (Adobe PDF)
  Itemized statements for the current semester can be accessed at the Student Online Center. For previous semesters, an itemized receipt can be re-produced for the following cost-per-receipt: [$3.00: Fall 2003 or later (5-7 Day processing time)], [$5.00: Prior to Fall 2003 (7-10 Day processing time)].
